Fertility Expenses The costs that you pay for
fertility testing and treatment of infertility for
you or your spouse are reimbursable under the Plan.
This includes the cost of drugs, shots, tests and
surgeries performed by a physician on you or your
spouse. This would not include any costs paid for
the use of a surrogate or the eggs of a surrogate
or donor.
